Community Connector- Social Prescribing

Through funding made possible by the Saskatchewan Seniors Mechanism, the City of Yorkton is establishing a pilot initiative for Social Prescribing for older adults (55+). This three year project funds a 20 hr/week contract position to implement social prescribing for older adults in the community.

The anticipated contract term for this position is February 2025 through September 2027.

Accountable to the Social Prescribing Steering Committee, the Community Connector accepts referrals from health care providers and other sources to connect one-on-one with older adults to empower them to take control of their health and wellbeing through a holistic approach by linking them with community groups and services for practical and emotional support.

In this role, the Community Connector will
• Promote social prescribing for older adults, and complete referrals connecting individuals to supports within their community.
• Provide personalized support to those within the older adult demographic.
• Support community groups and organizations who serve older adults.
• Work collectively with local partners to ensure strong and sustainable supports for older adults in the community.

• Experience in project/program management
• Ability to listen and empathize with individuals
• Commitment to reducing health inequalities
• Demonstrates personal accountability and considerable self-initiative
• Ability to follow and create policies and procedures (e.g. confidentiality)
• Computer knowledge and experience with data collection
• Commitment to personal and professional development
• Knowledge of the local community is considered an asset
• Access to own transportation, including a valid class 5 drivers license
• Ability to work independently and with diverse members of the community
• Clear understanding of advocacy
• Updated Criminal Record Check and Vulnerable Person Check
• Ability and willingness to work flexible hours in order to meet work demands

Preference may be given to individuals who have worked with an aging population. Experience working with individual case management and care planning will be considered an asset.
